We need to do an insurance eligibility check before schedule your acupuncture with insurance initial appointment. Therefore, please send the following information ONLY to info@tcmfit.com for the check.

• Insurance plan name.
• Insurance member ID.
• Patient’s full name.
• Patient’s date of birth.
• Primary care physician referral letter (photocopy)
• photos of the front and back of your insurance card.

Please complete the designated forms to avoid any delay on your treatment.
